How To Become A Member

Membership intake is every Tuesday,  Wednesday and Thursday from 10:00 a.m. – 2:00 p.m.  To join, simply click the button below and complete the registration form and return completed form to the center signed by your physician.

ELIGIBILITY
How to Become A Member

1. AGE
The minimum eligibility age requirement is 55 years with the ability to care for yourself.

2. HEALTH
Members must exhibit independence and ability for self-care. Mental disorientation or physical disabilities requiring individual care cannot be accommodated. LWSC and Human Services Department will exercise discretionary judgment as to the center’s ability to accommodate any individual. The Department reserves the right to decline services to individuals if the staff and/or facilities are inadequate for their needs. In such cases, the Department will suggest appropriate resources to caregivers.

3. OUT-OF-COUNTY PARTICIPANTS
The Lou Walker Senior Center was built and maintained for the senior citizens of DeKalb County at this time.
Out-of-County participants are permitted if they are a member of the Silver Sneakers program.

4. MEMBERSHIP FEES
Lou Walker Membership Fee: $120.00 annual fee (Example of the 12-month Annual Membership period: Paid July 2016, ends July 31, 2017).

II. Forms of payment
Acceptable forms of payment are:

  • Credit Cards
  • Personal Check
  • Cashier’s checks
  • Money orders
    These need to be made payable to the Lou Walker Senior Center.

PROCESS FOR ALL NEW/RETURNING MEMBERS

1. Completion of LWSC Medical Release Form
The form can be picked up from the center or from the LWSC website. The form needs to be completed by a physician.

2. Completed Health History Form
The Health History form needs to be completed by a physician. The potential member will bring the completed forms to the LWSC front desk and they will receive an LWSC Membership Packet, which will contain a registration application, Emergency contact sheet, and an income verification form. Membership intake is on every Tuesday, Wednesday and Thursday from 10:00 a.m. – 2:00 p.m.

3. All new and returning members must meet with a Lou Walker Senior Center social worker.

4. Once all the paperwork is completed, payment of $120.00 is required.

5. The member will have his/her picture taken and assigned an LWSC Membership ID card. LWSC ID card must be scanned and worn each day you attend Lou Walker Senior Center.

6. Congratulations! You are now an LWSC member.

40,000 Square Foot Multipurpose Facility

Our 40,000 square foot multi-purpose facility features a therapeutic Pool, computer lab, full-service kitchen with Piccadilly on site; 59 arts & crafts and other Lifelong Learning Classes, 53 aerobics and land fitness classes, 18 technology classes, including digital camera workshops; and classrooms and resource rooms offering a range of educational and other information.
